My husband and I concocted this survey to get some tips/advice about parenting our ds 7 mo and in order to better understand our own parenting future.

That's a clever idea.
You have to do something about the "How do you define yourself Jewishly" question ... Maybe you're better off just leaving it open-ended |

Thanks for the feedback. It's been tweaked :-) Hopefully I'll be able to share results with you if we get a worthwhile amount of responses. We're kind of excited about this
