Here's my attempt. I've written the chords in the verse/intro as full barre chords, it doesn't really sound like Eriko plays them like that, but they
are the right chords, she just seems to do a lot of playing around with different inversions/not playing the full chords and positions, which sounds great but is a nightmare to tab.

It’s really hard to tell what she’s actually playing here because the guitar, bass and piano are all playing similar things, moving about in harmony. This just about works, the bracketed note is kind of an example of a harmony you could add in to fill it out like the recorded version.
I might get around to doing a bass tab sometime, since the bass does a lot more interesting stuff than the guitar most of the way through the song.
