Concerning Power-Ups. The amount Power is raised by power-increasing items is as follows:
Common: 20%
Uncommon: 30%
Rare: 40%
Super Rare: 50%
Ultra Rare: 60%
Thus, for example, the move Heat Wave has a Power of 100. With a Charcoal (common) this would rise to 120. With a Flame Plate (rare), 140, etc. ^^ Likewise, the move Bonemerang has a power of 50. So a Brown Scarf would make it 60, or an Earth Plate 70. For more complicated numbers, use your computer's calculator. Also, concerning decimals that may occasionally arise, .5 or more is rounded up, .4 or less is rounded down. ^^

Raticate Whiskers: Stiff whiskers that can be used for many things, most popular thing is to use them to tie objects together.
Stunky Musk: A mild repel, decreases encounters with Pokémon.
Uncommon: Deep-sea scale: Clamperl's Special Defense increases (evo item)
Deep-sea tooth: Clamperl's Special Attack increases (evo item)
Bright Powder: Raises Evasiveness
Everstone: Prevents user from evolving when equipped
Focus Sash: Prevents one-hit-KO moves from defeating holder, may only be used once
Lagging Tail: Holder always attacks last
Lax Incense: Lowers enemy's chance of landing a Critical Hit
Lucky Punch: Critical Hit ratio rises
Amulet Coin: Increases chance of finding common & uncommon items.
Muscle Band: Slightly raises power of Physical moves
Mental Herb: Removes Infatuation status; can only be used once
Blaze Orb: Raises power of fire-type moves
Blackglasses: Any Dark Type Move's Power is increased. Plus they look cool.
Macho Brace: Raises power of fighting moves
Miracle Seed: Any Grass-type move's power is increased
Mystic Water: Any Water-type move's power is increased
Nevermelt Ice: Any Ice-type move's power is increased
Poison Barb: Any Poison-type move's power is increased
Rock Incense: Raises power of Rock attacks
Sea Incense: Any Water-type move's power is increased

Bone Club: Can be used as a weapon; Gives Bone Club attack (doesn't use up an attack space)
White Herb: Cures any status effect; can only be used once
Venusaur leaves: Healthy looking leaves that had been shed from a Venusaur, they give some protection from the rain. Also good for eating. [uncommon]
Skuntank Musk: A strong and powerful repel, the chance of being attacked or meeting anyone is very slim.
Batwings: Treated Golbat wings. These allow you to glide distances and make you look a little spooky ^^
Zubat Fangs: A pair of Zubat Fangs. They give you the move Leach life, although they can make speaking a little hard.
Water Stone: These are made by rivers and, as such, can be found at pretty much any major water way. Evolves those that need it.
Moon Stone: These can be found enmass in the Land of the Daikaiju. Can also be taken from a Lunatone if you manage to chip it, but they are very rare and don't like parting with bits of themselves, for obvious reasons. Evolves those that need it.

Adamant Orb: Raises the power of Steel and Dragon-type moves
Lustrous Orb: Raises the power of Water and Dragon-type moves
Platinum Orb: Returns Giratina to Origin Forme when attached, boosts Dragon and Ghost type moves.
Glacidia Flower: Allows Shaymin to use Sky Forme.
Soft Sand: Any Ground-type move's power is increased
Spell Tag: Any Ghost-type move's power is increased
Focus Gem: Any Psychic-type move's power is increased
Light Clay: Prolongs effect of Reflect and Light Screen
Luck Incense: Increases chance of finding rare items
Smoke Ball: You can run from any battle without fail; can only be used once before needing a refill
Poison Thorn A thorn from a Rosalia, it contains enough poison to make someone sick for a long while.
Leak: An onion plant, tastes great. Just don't let any Farfetch'd see you with it.
Tauros Horn: A horn that ranges in size according to the creature's age, only way to obtain these is to kill a Tauros and that is no easy feat.
Smeargle's Paints: A set of 5 colors of paint, stolen from a Smeargle. The paint, if applied to yourself, will stay on you for about a year.
Eevee Tail: A soft tail, the remainders of a kill possibly. Great for a pillow and a self insulin.
Mightyena pelt: A beautiful, shiny pelt of a Mightyena. It's sad this graceful creature was killed, but it offers warmth. You gain the ability Intimidate from time to time if you wear it like your own skin.
Thunder Stone: Evolves Pokémon that need it. Do you know how hard it is to bottle lightning? Let alone turn it inot a gem...
Grass Stone: Actually part of an ancient, fossilised Leafeon, usually the ear or tail. Evolves those who need a grass stone to evolve.
Fire Stone: These are commonly found near Volcanos, especially after an eruption. Evolves Those that need one to evolve.
Sun Stone: Acutally a broken-off part of a Solrock. Don't worry, they grow back ^^ Evolves those that need one.
Aura Stone: Raises all stats of any Psychic or Fighting type. Increases the power of Aura-based attacks by 50% (rounded to the nearest ten. Aura attacks include all attacks that have Aura in the name).
Super Rare: Grip Claw: A trap that stops opponent's from moving.
Life Orb: Increases attack damage but decreases HP whenever you deal damage
Lucky Egg: Increases chance of finding any item
Scope Lens: Increases chances of landing a Critical Hit with non-physical attacks
Togekiss Feather: Gives you the ability to levitate for a short time.
Blissey Egg: Said to bring unlimited happiness to anyone who holds it, it's good luck. It may be able to save you from a tough situation. It spoils after 2 weeks.
Dragonair Orb: It glows nicely if you shine it, in the hands of a kind hearted Pokémon it grants you a wish.
Absol Scythe: After a long life, an Absol willingly allows someone to cut off the scythe-like appendage on it's head. It can be used as a weapon or a keepsake. It's said that the soul of the Absol will travel with you. Gives the moves Razor Wind (to begin with) and Night Slash (at later levels, determined by mod).
Ultra Rare
Uber l33t H4xs0r: This... odd item was said to have been made by an insane PorygonZ. It has no defined shape, as it seems to be made entirely of Porygon data, though it mostly appears as a sword of some kind and always has the colours of a Porygon. It is said to be able to power-up any special attack by 100% (unique: no current owner)
Fists of Ultramar: A pair of steel, mechanical gauntlets, made by the Zion Matrix clan based upon an ancient human role playing game. Can power up any physical attack by 100% (unique: no current owner)
Helm of Arceus: An ancient helmet said to have been made from the essence of Arceus. Can change a character's typing to any other type just by a force of will. (unique: no current owner)
